Our most common cause of failed AD logins is some sort of corruption that occurs in the cached AD account in NetInfo DB. We find that there will be two to three instances of the cached credentials, and one will be disabled. After we delete that instance, the user then can log in normally. We are still investigating why this occurs, and would love to get some input.
